Augment Health Solutions Pty Ltd (trading as Atidia) secure, internet based health communication software platforms, and related services, was developed by doctors, for the use by patients, hospitals, doctors, nurses and administrative staff. Our products are utilized by healthcare institutions, healthcare providers and similar institutions to allow them to provide health services, advice and communications to the patient in order tom optimize their care.
The information you provide can help identify what is the most appropriate, safe and effective treatment and care for the patient (you). We comply with our privacy and confidentiality obligations.
We understand and respect the importance of the doctor patient relationship and the significance of keeping your personal information, particularly your health information private, secure and confidential.
This privacy policy outlines how Augment Health Solutions (AHS) Pty Ltd ABN 19 635 412 467 and its related bodies corporate and associates (we, us or our) collects, holds, uses and discloses personal information as required by the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (Privacy Act).
In this privacy policy, Augment Health Solutions means the websites operated by us, the Augment Health Solutions mobile applications and platforms made available by us, and/or any other associated services provided by us.
Kinds of personal information we collect
Personal information about patients and caregivers
Personal information identifies you as an individual whether you are a user, patient, health professional, customer and potential customers, contractor and other people who come into contact with us (you).
Personal information is defined as “ any information and opinion about you where your identity is apparent or can reasonably be ascertained” The kinds of personal information we may collect and hold may include your name, address, phone numbers, e-mail address, geographic location and Health Information.
Health information is all identifying “personal information”, collected to provide a health service. The APP’s health information comes under the definition of “sensitive information”.
Any other information you provide to us when you use Augment Health Solution products, including which of our pages or other webpages you have visited, as well as how and what you use Augment Health Solutions for is also considered personal information.
Personal Information from providers
If you are a physician or other healthcare and or medical provider (“provider”) or an individual who is authorized by a provider or healthcare institution to access and use the service (“designee”) we collect without limitation, about providers and designee’s, name, specialty, email address and phone number. We do not collect health information about the provider or designee.
How we collect personal information
We will generally collect personal information about you directly by way of forms and other documents or information you submit to us (electronic form) such as questionnaires, correspondence you provide to us and telephone calls or in person meetings with you.
We may also collect any information and data uploaded or submitted to us in connection with your use of Augment Health Solutions products. Information from your referring physician and other specialist as required may also be collected to ensure a full picture of your medical history.
If you are a Patient, this may include health information and other personal information about you entered into Augment Health Solutions products and questionnaires or otherwise submitted to us by you or your health professionals and or caregiving team including any designee. However, we will only collect health information from Patients that have been registered to use Augment Health Solutions (wetherby health professional or their designees) and that have provided us with consent to collect the Patient’s health information.
How we use personal information
We will only use personal information for the following purposes unless otherwise required or permitted by law:

To allow your health professionals and any designee to communicate with you about your care.

To allow your health professional to give you health advice.

To allow any designee to give you information and advice regarding your treatment plan.

To allow you to share health information and other information about you with other Augment Health Solution Pty Ltd users (“providers”) and (“designee”) that you have already authorised to access such information (e.g. health professionals providing you with health care services) or otherwise.

To send correspondence to your referring practitioner and or nominated practitioner, specialists and any healthcare practitioner involved in your care.This is in accordance with health industry accepted practice and will inform them of any ongoing care required, changes in care and any follow up required.

For our internal management and business such as data analysis, audits, developing new products.
For other purposes which are reasonably necessary in connection with our normal functions and activities. If we are unable to collect personal information relating to you, or to otherwise continue our relationship with you.
Disclosure of your personal information
We may disclose personal information to the following entities for the purposes mentioned above; our contractors, consultants, advisors, hospitals, healthcare professionals, associates and related entities; if you are a Patient, to your health professionals; any industry body, tribunal, court or otherwise in connection with any complaint made by you about us; if you have provided us with referees to assist with the assessment of a potential contract between you and us, the referees you have provided; and other entities with your consent or as permitted or required by law. As at the date of this privacy policy the recipients are located in Australia, although the countries in which the recipients are located may change over time.

We hold personal information in electronic form. We have in place the very best steps to protect the personal information we hold from misuse, interference and loss and from unauthorised access, modification or disclosure which are above the requirement of Australian standards.
How to obtain access to your personal information
You may obtain access to personal information which we hold about you by contacting us using the contact details set out below. When you request copies of your personal information held by us we will provide you with such personal information as soon as reasonably practicable.
We will require you to verify your identity and specify what information you require. There may be occasions when access to personal information we hold about you is denied. Such occasions would include where release of the information would have an unreasonable impact on the privacy of others.

By your provider utilizing Augment Health Solutions Pty Ltd services, there is an implied consent for you to receive an SMS request for information. We may also send you reminders regarding your medical treatment, advice and follow up of your care.
How to make a complaint about a breach of your privacy rights by us
If you are of the view we have breached the APPs, the Privacy Act, or any related privacy code in dealing with your personal information, you may make a complaint by writing to us using the contact details below and we will respond and provide you with confirmation as to how we propose to deal with the complaint as soon as reasonably practicable.
